On the accreditation form, under <i>Broad description of the activity</i> there is an option to select LIA/III Accredited event.

 For LIA/III accredited events you will be asked to provide the accreditation confirmation detailing the hours awarded by the relevant institution. 

You will need to complete all fields of the form to submit the request. If they are not relevant for you, please include N/A.
